The goal of the Passport Foundation’s Impact Fund Program is to sponsor grassroots initiatives that can demonstrate evidence-supported improvements in health outcomes for Kentuckians. Please review our funding priorities for more information.

  • Grassroots initiative examples may include (but are not limited to): community-level research and engagement, program startup expenses, small equipment, and/or essential program supplies.


  • Passport Foundation will not consider applications for fundraising events, political or religious initiatives, and/or capital campaigns.
  • Please note that funding through this program is separate from what your organization may bill to Passport Health Plan for contracted provider services.
  • All applications must be completed in full or your application will be automatically denied.
  • The Passport Foundation will not fund organizations that discriminate based on race, color, religion (creed), gender, gender expression, age, national origin (ancestry), disability, marital status, sexual orientation, or military status, in any of its activities or operations.
