At the Passport Foundation, we recognize that in order to improve the health of our members and their communities, we must invest in programs that fortify resources, resilience and relationships to make a lasting impact in areas of Kentucky that need it most.

Passport Health Plan members live in every corner of Kentucky and many face non-medical challenges that directly affect their health, including but not limited to:

  • Limited access to fresh food and/or reliable transportation,
  • Inequitable access to health care resources for growing families,
  • A lack of educational resources that lead to fulfilling and well-paying jobs, or,
  • An addiction to drugs or alcohol left untreated.

Through its Grants program, the Passport Foundation supports non-profit organizations and partnerships to help them better carry out their missions of improving the lives of Kentuckians.
