Passport Foundation, Inc.is a 501(c)(3) public charity established in 2017. We are the sister organization and philanthropic arm of Passport Health Plan, a 20-year old nonprofit, community-based health plan administrating Kentucky Medicaid benefits.
The Passport Foundation was formed to close the gap of coverage of services for non-clinical aspects of Passport’s members and others in Kentucky.
The Foundation’s main goal is to raise funds to support programs and partnerships that improve social outcomes by investing in community programs and partnerships throughout the state. It is simultaneously developing community-based programs and seeking funding for their implementation. Development of programs is evidence-based and guided by the leadership of the Passport Foundation and its Board of Directors.
Good health has long been defined as simply not being sick, yet health and well-being are also strongly influenced by complex social factors such as where we live, how we work, the safety of our neighborhoods, and the resilience of our families. In fact, research indicates that traditional clinical care accounts for just 20% of health outcomes.
New and innovative non-clinical approaches to health and well-being are starting to provide an impact on what happens outside traditional medical systems with a focus on those living in Kentucky who are most vulnerable to risks. This is what we are working towards every day at the Passport Foundation.
